Your Responsibilities

It is your responsibility to make sure we have updated address, phone, and insurance information. We are not responsible for contacting you in a timely manner if your phone does not accept blocked calls. Bring your insurance card with you to each visit. Patients will be asked to reschedule if an insurance card is not presented at the time of service. It is your responsibility to know the details of your insurance plan, such as whether you need referrals. Let us know if you special requests as to how you want your visit billed.
